Requesting Records

Access to Information/Disclosure

If you are currently receiving services from Durham CAS or have in the past, and would like access to your personal information, please fill our Access to Information/Disclosure form (*form opens in new window and is submitted on website).

You must attach one copy of valid government issued photo identification along with your request. Please note, the identification of any person age 16 years and older is required to release their information and must be included with your request. If you or any person age 16 years and older do not have identification, please note this in the Request Description field on the form.

A person with custody of a child under the age of 16 may request personal information of that child, subject to some exceptions. In some circumstances, you may be asked to provide documents confirming your authority to make the request in place of the child.

Completion of the Access to Information/Disclosure request form indicates that each party listed consents to the sharing of their personal and/or relevant information with the other consenting parties named on the form.

Child Welfare Record Check

If you require a Child Welfare Record Check for employment, please fill out the Child Welfare Record Check form (*form opens in new window and is submitted on website).

You must attach one copy of valid government issued photo identification along with your request. This request should be made to the child protection agency in your place of residence, including past residences. Durham CAS cannot provide child welfare information from another Society.

Adoption Disclosure

If you were adopted or related to someone who was adopted through Durham CAS and you would like to request non-identifying information, please fill out the Adoption Disclosure form (*form opens in new window and is submitted on website).

If you are a descendant of an adoptee who is deceased, a copy of the death certificate will be required. You must attach one copy of valid government issued photo identification along with your request.

For copies of birth registration and adoption orders for all adoptees, you must apply to the Ministry of Children, Community and Social Services at Service Ontario or toll free at 1-800-461-2156.

Record Correction

If you have previously been provided with access to a record of your personal information and believe that the record is inaccurate or incomplete, please fill out our Record Correction form (*form opens and is completed and is submitted directly through website).

You must attach one copy of valid government issued photo identification along with your request.
